Consumer Directed Services

Need assistance with housekeeping, cooking, grocery shopping, bathing and or transportation?   The CDS program may be for you!  If you are interested to see if you qualify or just want some information, give us a call!  You can pay a family member or a friend to take care of you!  No Strangers!  Pay range is $10.50 - $12.00 per hour.         

Requirements for CDS;   

•Ability to self-direct (direct the care planning process and hire an attendant)   

•Must have a physical disability   

•Must be at least 18 years of age    

•Must be Medicaid eligible and active     

•Must have a personal care physician (family doctor)    

•Must meet level of care through Department of Health and Senior Services
